We have two order forms – one for your Artists Gallery Life Shade™ selection and one for your Custom Life Shade™.
Custom Life Shade™
Please take a minute to describe your custom order using our custom order form. If you do not have a custom order form, download one using the link below, print it out, fill it in, and fax it to us. The order form has all of the prices plus the details we need to create your personalized Custom Life Shade™. Alternatively, you can call our offices at (540) 661-3434 or email us at sales@lifeshade.com to request an order form. We accept most major credit cards and PayPal.
The Custom lamp shade Order Form allows you to personalize a lamp shade with up to four of your own photographs. Simply specify one photograph for each side (panel).
We accept digital photos, slides, or traditional prints.

Wholesale Life Shade™
Wholesale Orders are accepted for approved retailers, professional photographers, artists, etc. who can provide a tax-exempt certificate and a business card. Please call our offices Monday through Friday, from 9 to 6pm EST for details on wholesale pricing and policies.
Be sure that photographs are clearly marked -- code their location on the printed piece and indicate to whom they should be returned.
SENDING Images
We accept computer files on CD Rom, as an E-mail attachment
(compress large files). Upload jpegs,
tiffs, or pdf file formats.
